Derek Fortman

OTTAWA – Derek Fortman, 33, of Ottawa died 3:02 p.m.  Sunday, September 24, 2017 at Ohio State University Hospital, Columbus. He was born March 3, 1984 in Van Wert to Jay A. and Donna (Closson) Fortman. His father survives in Ottawa, his mother, now Donna (Todd) Worchuck survives in Ottawa.

He is also survived by four daughters: Sadie Fortman, Ella Fortman, Lily Fortman, Aviana Fortman all at home; a son, Gabriel Fortman at home; four brothers: Jason (Jessica) Fortman of Defiance, Jay Fortman, Jr. of Ottawa, Travis Fortman of Wapakoneta, and Caleb (Victoria) Davis of McComb.

He was preceded in death by his grandparents, Fred and Rose Fortman and Richard and Rozella Closson.

Derek worked for Waste Management, Lima. He was an avid music lover and the most important thing to him was his children.

A Mass of Christian Burial will be 10:30 a.m. Friday, September 29, 2017 at Sts. Peter and Paul Catholic Church, Ottawa with Fr. Matt Jozefiak officiating. Burial will follow in the church cemetery. Visitation will be from 2 to 4 p.m. and 6 to 8 p.m. on Thursday at LOVE FUNERAL HOME, Ottawa where there will be a scripture service at 7:45 p.m.
